چکیده

In a previous work we started the study of the influences of different methods of collapse-therapy-and particularly pneumothorax-on the respiratory function of each lung. We are presenting here the results of the air content analysis of each lung before and after the surgical paralysis of the phrenic nerve. Oianotti and Ceruti2 have seen in tracheotomized dogs that the ventilation of the lungs and the oxygen consumption diminishes after the excision of the phrenic nerve and that these effects persist after several weeks. The reduction in the respiratory values is somewhat higher in the bilateral than in the unilateral paralysis. Brea and Ferrari3 made a functional study of seven cases of pulmonary tuberculosis before the paralysis of the phrenic nerve and also a few days and again one month after the surgical paralysis (by excision, alcoholization or crushing). After a few days they found a diminution of the voluntary apnea, the vital capacity, the respiratory volume per minute, the oxygen consumption, CO., elimination, and in the O., content of the alveolar air. Due to the development of compensatory mechanisms all these changes return to normal values one month after the intervention. Cournand and Richards: in eight cases of therapeutic phrenic paralysis found striking impairment of the pulmonary function, as evidenced by decreased breathing reserve, unsaturation of arterial blood and a distinct tendency to pulmonary congestion after intravenous infusion. According to Rossier, Petzold, Decker and Michaud, cited by Steh~l in ,~ the paralysis of phrenic nerve results in an unsaturation of arterial blood during moderate work and sometimes even during rest. To explain this finding it is suggested that although there is a reduced ventilation, the amount of blood circulating through the lower part of the homolateral lung remains constant. A pendular phenomenon in the air of lungs due to the paradoxical movement of the diaphragm has also been described.

Phrenic and diaphragm function after coronary artery bypass grafting.

We studied respiratory mechanics and phrenic nerve and diaphragm function in 12 patients on the day before and eight to 13 days after coronary artery bypass grafting. The average vital capacity, functional residual capacity, and total lung capacity decreased by 20.5%, 9.5%, and 14.7% respectively after operation. Eleven patients showed less negative maximum inspiratory mouth pressures at any gi...

Diaphragmatic Plication for Patients with Acute Phase Phrenic Nerve Paralysis Following Lung Transplantation: A Case Report

Phrenic nerve injury can occur as a complication of lung transplantation. A 54-year-old man underwent single-lung transplantation due to interstitial pneumonia. The patient required circulatory support with venoarterial extracorporeal membrane oxygenation and was unable to be weaned from ventilatory support with nitric oxide. Although enhanced CT scanning showed stenotic anastomosis of the righ...

Shortness of breath after AV ablation: case of left phrenic nerve palsy

Phrenic nerve palsy has been recognized as a complication of catheter ablation with a prevalence of 0.11-0.48% after atrial fibrillation ablation, independent of the type of ablation catheter or energy source, likely due to the anatomical relationship of the nerves. This report describes a case of new onset of shortness of breath (SOB) due to left diaphragm paralysis following transcatheter rad...

Bilateral Facial Paralysis and Otitis Media as the First Presentations of Wegener’s Granulomatosis: A Case Report

Introduction: Cranial nerve palsy in Wegener’s granulomatosis is a curious incident, particularly if it occurs without kidney or lung involvement. In a review of medical articles, only 1 case of Wegener’s granulomatosis with bilateral facial nerve palsy was found. Case Presentation: The patient was a 16-year-old female who presented with pain and hearing ...

Persistent phrenic nerve paralysis following interscalene brachial plexus block.

A 60-yr-old man, 160 cm tall, weighing 75 kg, with American Society of Anesthesiologists physical status class II, was admitted for elective right shoulder surgery. His medical history was unremarkable except for recent mild diabetes with no related neuropathy, controlled by diet and glimepiride. Physical examination results were unremarkable, and the results of laboratory studies were all with...
